Participation and Membership

HEALTH-CARD-300x190 Participation in our Loyalty Program is available to individuals only. An individual can have upto 4 family members as add-on on the card. If you wish to become a member of our Health Card Program, you can complete our membership process in person at our head office at 16/1 Race Course Road, Indore. You will be bound by these terms and conditions when you first sign up to join our Health Card Program. All members will be assigned a membership number which will be used by Nidan Diagnostic Centre for identification purposes. You must produce your membership card each time you visit any of our pathology. Only members and their add-ons are eligible for program benefits..

Eligibility

To be eligible to participate in our Health Card Program, we will issue you with a membership card. Even if you do not receive a membership card, you are still eligible to participate in the Health Card Program provided you quote your membership number.Once you are eligible to participate, each time you visit any of our pathology, you are eligible for a discount of 10% on all investigations except on the services listed under negative list. No discount would be offered on negative list. Elite members will get additional benefits.The membership card remains the property of Nidan Diagnostic Centre and is not transferable. If your membership card has been lost or stolen, please notify us immediately to enable us to issue you with a replacement card. A fee would be charged to issue replacement card. Nidan Diagnostic Centre may at its discretion cancel your membership or refuse you membership to its Health Card Program, at any time without notice to you.
